There was nothing whatsoever inadvertent about what Trump said or did. It was not a gaffe under any definition, except perhaps in the Water-Carriers Dictionary.

I never said it was. If anyone thought I said that his rude language was a gaffe in that it should not have been made to anyone, it was purely due to their lack of comprehension. Only the making it to the wrong person was a gaffe.
Nor was calling making the insult to the wrong person a gaffe, which it was, a defense of Trump.
It seemed, however, that the OP thinks the critical issue was that the insult went to the wrong person.
