When you are constantly pushing the very edge of military tech it simply isn’t avoidable.
When projects take years or decades to complete it’s unavoidable.
Carriers are the equivalent of floating cities and military bases combined. They are the most complex weapons delivery system ever developed and we’ve always has similar problems with them.
Answering just your first sentence, just because it has happened before or indeed happens frequently is a **** poor reason for excusing this crap when it happens again.
And the latest failure is NOT a first in class issue, it is the failure of a fundamental part that is not particularly cutting edge. We should not pay for this, the builder should eat the cost for this in full.
