A “lone gunman” can be tacitly developed, monitored, and allowed to get a shot. Incompetence and communication failures are the excuse. That appears to be what has happened in multiple US assassinations and attempts.

Alternatively foreign allies can do the dirty work–a covert killing of an American in the US is not a violation of foreign laws. The FBI and other federal agencies simply allow things to happen and allow the guilty parties to flee the country. That appears to be what happened with 911.
